Utah tick submission form
Thank you for your interest in Utah's tick surveillance efforts! The Utah Department of Health and Human Services (DHHS) is interested in collecting tick data from the public to better understand when and where ticks are found in Utah. Your tick submission helps DHHS in this statewide effort. We appreciate your contribution!
Anyone who wants to submit a tick to contribute to our surveillance efforts should use this form. Completion of this form is voluntary. If you give us an email address, we'll send species identification results to you. Pathogen testing will be attempted on ticks that are submitted to the Utah Public Health Laboratory (UPHL), but results cannot be guaranteed. Individual results will not be shared with the submitter. Instead, aggregate statewide data will become available when results are confirmed. Tick testing should not be used to determine an individual's need for treatment. Anyone who believes they have been bitten by a tick should watch for symptoms and seek medical care if they become ill.

If you have a tick (or ticks) you would like to submit, fill out this form for each tick group. For the purpose of this survey, a tick group is defined as ticks collected on the same date and/or from the same location (i.e., found on the same people, animals, or trails). After you complete the survey, reach out to the one health team (onehealth@utah.gov) to coordinate getting the tick(s) shipped to UPHL.
