Welcome to the School and Childcare Immunization Reporting System.

The immunization reporting deadline has changed from November 30 to January 30 each year. For the current school year, the report is due on January 30, 2025. If you have questions about completing the report, please email Nasrin Zandkarimi at nzandkar@utah.gov or call 801-538-6570.
